What the template covers
Cleaning frequency, each system component (hood interior, grease filters, plenum, horizontal and vertical duct, exhaust fan) marked cleaned, inaccessible or not applicable, whether access panels are adequate, deficiencies found, the service sticker, photos and signatures.
State what you could not clean
A report that says “all clean” when a vertical duct had no access panel is the one that causes trouble after a fire. Mark it inaccessible, say why, and recommend the panel.

How often should kitchen hoods be cleaned?
NFPA 96 ties frequency to cooking volume: monthly for solid fuel, quarterly for high volume, semi-annually for moderate and annually for low volume. Your local AHJ has the final say.
Is this a hood cleaning certificate?
It is a service report. Many jurisdictions also require a sticker on the hood; some require online filing.
